Dermal filler longevity varies significantly by treatment area and filler type. As a general guide: lips last 6–9 months, cheeks 12–18 months, jawline 12–15 months, and under-eyes 9–12 months. Your individual metabolism, lifestyle, and the specific product used will all influence how long your results last — and a skilled physician can help maximise that duration.
Understanding How Fillers Work
Most modern dermal fillers are made from hyaluronic acid (HA) — a naturally occurring sugar molecule found in the skin, connective tissue, and eyes. HA fillers work by attracting and binding water, adding volume, hydration, and structure to the areas where they are placed. Because HA is biocompatible and eventually broken down by the body's own enzymes, it is both safe and temporary.
Non-HA fillers — including calcium hydroxylapatite (Radiesse) and poly-L-lactic acid (Sculptra) — stimulate collagen production and typically last longer, often 18–24 months. At Aurelia, we select the most appropriate filler technology for each patient's anatomy, goals, and desired duration of results.
Filler Duration by Treatment Area
The following timeline reflects typical outcomes at Aurelia with premium HA fillers and proper injection technique. Individual results vary.
| Treatment Area | Typical Duration | Notes |
|---|---|---|
| Lips | 6–9 months | High movement area — muscle action accelerates breakdown. Softer, more flexible HA formulations are used here. |
| Cheeks & Midface | 12–18 months | Lower mobility means filler breaks down more slowly. Firmer, more structured products are ideal for lifting and contouring. |
| Jawline | 12–15 months | A dynamic area subject to chewing and speaking; slightly shorter duration than cheeks but amenable to annual maintenance. |
| Under-Eyes (Tear Trough) | 9–12 months | A delicate area requiring thin, hydrophilic fillers. The under-eye is relatively low-movement, supporting good longevity. |
| Nasolabial Folds | 9–12 months | Active facial expression reduces longevity somewhat. Medium-density fillers work well here. |
| Nose (Non-Surgical) | 9–12 months | Precise placement is critical in this area. Results are subtle but effective at refining bridge and tip. |
| Chin | 12–18 months | Low-mobility zone. Structured filler provides excellent projection and definition for up to 18 months. |
| Temples | 12–18 months | Excellent longevity in this area; often overlooked but highly effective for facial frame restoration. |
| Hands | 9–12 months | Subject to frequent use; Radiesse (CaHA) is often preferred here for its collagen-stimulating properties. |
Factors That Affect How Long Fillers Last
The numbers above are averages — and individual variation can be significant. The key factors that influence how quickly your body metabolises filler include:
1. Metabolism and Lifestyle
People with faster metabolisms — often those who are very physically active or have lower body weight — tend to break down hyaluronic acid more quickly. High-intensity exercise, in particular, increases circulation and metabolic activity, which can reduce filler longevity by 20–30% compared to someone who is moderately active.
2. Area Treated
As shown in the table above, areas with high muscle movement metabolise filler faster. The lips, which move constantly during speaking and eating, see the shortest duration. The temples and chin — relatively static zones — can hold results for well over a year.
3. Type and Brand of Filler
Not all hyaluronic acid fillers are equal. The degree of cross-linking (how the HA molecules are bonded together) significantly affects how long the product resists enzymatic breakdown. Highly cross-linked fillers like Juvederm Voluma or Restylane Lyft last considerably longer than softer, less cross-linked products used in the lips. At Aurelia, we use a curated range of products from Allergan and Galderma — both leaders in filler technology.
4. Injector Technique and Placement
Placement depth matters. Filler placed in the correct anatomical plane — typically the deep fat compartments or subperiosteal (bone) level for structural work — lasts longer and behaves more naturally than filler placed too superficially.
5. Volume and Frequency of Prior Treatments
Patients who maintain regular filler appointments often find their results last progressively longer over time. This is partly because residual product accumulates, and partly because regular maintenance prevents the degree of volume loss that requires more product to correct.
How to Extend Your Filler Results
Tips to Maximise Longevity
- Use daily SPF 30+. UV exposure degrades hyaluronic acid and accelerates the breakdown of filler. Consistent sun protection is the single most evidence-based way to extend results.
- Stay hydrated. HA is hydrophilic — it binds water. Adequate hydration helps the filler perform optimally and may slow its breakdown.
- Avoid excessive heat. Saunas, steam rooms, and hot yoga increase metabolic activity at the tissue level and may accelerate filler degradation. Avoid for at least two weeks post-treatment.
- Don't smoke. Smoking impairs collagen synthesis and accelerates skin ageing, counteracting the benefits of filler treatment.
- Consider skin boosters alongside fillers. Treatments like Profhilo or Juvederm Volite improve the quality of the tissue around your filler, helping it integrate better and last longer.
- Schedule maintenance before it runs out. Topping up filler while some product remains — rather than waiting until it has completely dissolved — typically requires less volume and prolongs a natural appearance.
When to Schedule Your Touch-Up
As a guide, we recommend booking your maintenance appointment when you notice you are approximately 60–70% of the way to your original result. At this stage, there is still product present to build upon, and the volume of filler required for the top-up is typically smaller than your original treatment.
For lips, this typically means a check-in at around six months. For cheeks and jawline, scheduling an annual appointment around the same time each year works well for most patients. Your Aurelia physician will advise you on the optimal schedule based on your response to treatment at your two-week review.
What Happens When Fillers Wear Off?
One of the most common concerns patients raise is whether their face will look worse once filler wears off than it did before treatment. The reassuring answer, with HA fillers, is: no. As hyaluronic acid gradually breaks down, the area simply returns to its pre-treatment baseline. You will not look "deflated" beyond your natural state.
Some patients also worry about filler causing permanent changes to the tissue. With correctly placed HA filler, there is no evidence of permanent structural change. The skin does not become dependent on filler, and the natural ageing process continues at its normal rate. If anything, the hydrating properties of HA may have mild positive effects on the surrounding tissue over time.
If you ever wish to reverse HA filler before it dissolves naturally, hyaluronidase (an enzyme that dissolves hyaluronic acid) can be injected to break it down quickly and safely — a significant safety advantage that HA fillers hold over permanent filler alternatives.
